To have received this allowance you would have to have had a disability which was recognised as very debilitating. All correspondence you receive from the DLA office - even if it's only the annual letter advising you of the increase in allowances - will state that you must inform them if your circumstances change. In the absence of more details, I am assuming that your disability is now not so severe (or has gone away totally) and that is why you wish to cancel receipt of the allowance. A phone call, or letter, to the DLA office stating that you wish to cancel receipt of the allowance will suffice. If you do not keep the letters they send, and therefore cannot find a phone number or address, this can be provided by your local post office or benefits office.
To continue accepting disability allowances when you no longer qualify for them is a criminal offence and could lead to a court case.
